Portfolio evidence examples
Evidence example 1: Abbreviations [samples and clinical]. This evidence piece gives an example of the use of some medical abbreviations found on request forms. The trainee has included objective evidence of a request form that has been annotated and then supplemented with some common abbreviations they encountered.
Evidence example 2: Phoning results example - the candidate has used a telephone log taken from the LIMS system of a result they gave out whilst under supervision. They have used a number of screenshots to explain the process step by step. The candidate has anonymised screenshots to remove any patient identifiable information - remember to mask data before scanning and the use of a black marker pen may not be sufficient.
Evidence example 3: Examples of internal communication methods used by the candidate. The candidate chose the following examples:

Daily Huddle form: used for daily internal meetings during handover / takeover between shifts.

Daily tasks sheets (e.g. logs, daily checks etc.).

Laboratory notice whiteboard.
The candidate used objective evidence from the workplace as evidence for each of the three chosen internal communication methods - annotated and anonymised. Portfolio v5 mandatory piece.

Each piece of evidence above also included mentor / trainee engagement feedback (e.g. ticks, short comments, follow-up questions). Signatures are available on originals but have been removed electronically from all above evidence examples.
