Portfolio evidence examples

Evidence example 1: Abbreviations [samples and clinical]. This evidence piece gives an example of the use of some medical abbreviations found on request forms. The trainee has included objective evidence of a request form that has been annotated and then supplemented with some common abbreviations they encountered

Evidence example 2: Phoning results example - the candidate has used a telephone log taken from the LIMS system of a result they gave out whilst under supervision. They have used a number of screenshots to explain the process  step by step. The candidate has anonymised screenshots to remove any patient identifiable information - remember to mask data before scanning and the use of a black marker pen may not be sufficient.

Evidence example 3: Examples of internal communication methods used by the candidate. The candidate chose the following examples:

  • Daily Huddle form: used for daily internal meetings during handover / takeover between shifts.

  • Daily tasks sheets (e.g. logs, daily checks etc.).

  • Laboratory notice whiteboard.

The candidate used objective evidence from the workplace as evidence for each of the three chosen  internal communication methods - annotated and anonymised. Portfolio v5 mandatory piece.

  • Each piece of evidence above also included mentor / trainee engagement feedback (e.g. ticks, short comments, follow-up questions). Signatures are available on originals but have been removed electronically from all above evidence examples.
