Portfolio evidence examples
Evidence example 1: Set questions. Although a simple evidence piece with only 2 questions asked, they allowed the trainee to highlight the variety of areas they worked in within their training period (both inside and outside of pathology).
Evidence example 2: Reflective Statement (on engagement with other service users). This piece was used as the mandatory piece of evidence, showing how the trainee has learnt with and from other professionals to not only benefit themselves but also the department and other service users that ultimately will improve patient care.
The candidate augmented the reflective piece with a piece of objective evidence relating to the area discussed during the reflection (sign off section of this reflective statement not included).
Another common evidence piece used for this mandatory section is the use of some type of user feedback questionnaire that is then reflected on. Portfolio v4 + v5 mandatory piece.
Evidence example 3: Feedback surveys. The candidate first of all provided context for why and how the feedback form was used, as well as how the feedback was collated from the questionnaire to produce a simple summary conclusion.
Note: if the candidate had reflected on this evidence piece it could have been used for the mandatory element for module 5. They instead decided to use it as a stand alone piece given the simple nature of the feedback questionnaire being used.
