Portfolio evidence examples
Evidence example 1: Category level 3 laboratory. This piece of evidence demonstrated facilities available for processing certain high risk [infecetion control] samples and is accompanied with a witness statement (not included below). The trainee has focused on the H&S aspects by explaining safety protocols when in use and how this equipment protects staff.
Evidence example 2: Laboratory reagents. Here the trainee has produced a list of all the reagents and chemicals they used whilst undergoing their training. They then identified the chemical hazards associated with each, along with an image hazard pictogram. Much of this data was taken from the relevant COSHH and safety data sheet information. During marking of the work, the mentor then asked some subsequent follow up COSHH questions that the trainee then answered (answers not included).
Evidence example 3: Pathology health and safety policy. The trainee has included an extract from the policy - the contents page that also includes the trust and document headers. The trainee has further explained the main areas covered by the policy and its scope. Again, after marking, the mentor has asked a number of follow up questions relating to the document (trainee answers not included below but follow up questions had also been marked by the mentor).
