Examples of paper-based data can include:
-
Sample request forms

Printed reports

Patient samples
-

-

1. Evidence one: x 4 examples of confidential data
-
Chose 4 examples form either list above (or you can choose you own examples) to complete the table for this evidence piece.
Evidence 2: For this piece of evidence you are required to describe how you would respond to a situation whereby a friend asks you to look up a result for them.
1. Evidence two: A friend requests to look up result information
-
Record how you respond to the above request (at least 3 paragraphs):
When writing up this piece it will help if you could answer the following:
-
How should / would you respond to such a request now and in the future?
-
Would you escalate / inform your line manager if you were unsure of what to do?
-
What further advice would you give your friend
-
Do you know what the term ‘confidentiality’ means?
-
Do you know what the term ‘professionalism’ means?
-
What could be the consequences of you ‘looked’ up some results for your friend?
Evidence 3: For this task you are to list 10 terms / abbreviations commonly used within your department and then state what this means and an example of when the abbreviation would be used.
Hint! Not all abbreviations used in your list need to be of a medical / clinical nature
1. Evidence three: x 10 terms / abbreviations examples
Can you think of anymore? (note: you can use examples 3, 4, 5 and 6 above in your table if you like)
Sign off: After you have collected each piece of evidence, hand them into your trainer / mentor who will then countersign them. Please note that they may ask for additional evidence or ask you follow up questions. Also ensure that:
-
You have date and signed your work
-
You only submit evidence for checking once it is completed (you may take individual evidence pieces out of your portfolio)
-
Ensure any countersigned checked work is returned back (in order) to your portfolio
Try not to hand in multiple pieces of evidence for marking at once - this will create too much work for your trainer / mentor, instead spread your submissions out throughout your training.
Trainer notes: Ensure the candidates line manager completes the 'Line Manager's Comments' section as each module in the portfolio is completed - continuous assessment of the candidates work is required throughout the training period.
