Portfolio evidence examples
Evidence example 1: HCPC standards set questions - Candidate has answered each question in one or two paragraphs max. The mentor has then asked 2 subsequent follow up questions after the initial work had been marked - highlighted in blue and green below (completed follow up questions not included). This demonstrates good mentor / trainee engagement and feedback.
Evidence example 2: HCPC standards of conduct. This evidence uses a HCPC website screenshot (to enhance the objective evidence piece) along with a hand written piece explaining these standards using personalised examples employed by the candidate. Note: colour coding annotated work helps break the work up making in easier for both mentor and assessor to understand.
Evidence example 3: Organisational chart with supplementary witness statement (signed off section of this witness statement not included). Candidate has annotated the organisational chart with further explanations of some of the roles.
