Portfolio past evidence examples
The examples below are for guidance only and give you some insight into the scope of the different types of evidence you may use – please remember work is not to be plagiarised.
The specific examples below are 'objective work-based' evidence types collected directly by the candidate from their work place, but remember to include other varied evidence types such as:

Witness statement / testimony written by another member of staff (e.g. observation of work place activities , performance and / or behaviours).

Reflective pieces (please use the reflection template that utilises the STEAM acronym).

Set questions that will require marking by your mentor.
The key points being that portfolio evidence should be varied in type, personalised and collected from your work-based environment as you progress throughout your training.
Finally, your mentor may ask for additional work to be completed (e.g. follow up questions or supplementary evidence). Do not rewrite your original work – it is best that you complete the additional work as a supplementary piece and attach with your original evidence piece (date and sign all supplementary work).
