Please find attached standardised questions we have set for your portfolio – once completed please hand into you work based mentor for marking.

 

You may also need supplement these questions with a small piece of objective evidence depending on your answers, but please seek advice from your mentor as we do not require lengthy pieces of evidences.

 

Your mentor will mark your questions and also provide you with some feedback – they may also ask you some additional questions, please do not rewrite the entire piece of work to include the additional questions. What we are looking for is interaction between the learner and mentor, so answer these questions as an additional piece of work (or annotate your additional answers by hand on original the original piece if short).
