Receiving Specimens
Evidence 1: Complete the table below describing the acceptance / rejection criteria associated with each of the following factors:

Evidence 2: Use a flow chart to describe the pattern of work flow for the receipt of specimens in the laboratory at North Tees – you could start with the following for example and then describe further how urgent and routine samples are then processed:

Once completed annotate the flow chart with the following information (if relevant):

Samples taken at Hartlepool or elsewhere that are tested at North Tees

When the peaks and troughs in work load occur

Staffing levels throughout the day / night
Evidence 3: Write a reflective piece on an untoward incident during sample handling that was brought to your attention - complete the following template ensuring all sections are completed. Once completed then submit as a piece of evidence directly to your portfolio.
Evidence 4: Complete an audit (using one of the Pathology QPulse audit templates) for any aspect or area of specimen reception.
Comment on any findings – where they what you expected?
Also remember to:

Have a conversation with your line manager in the first instance

Allocate time to conduct the audit

Allocate time to write up your audit and any findings

Discuss your findings with your line manager
Include this audit directly into your portfolio as an evidence piece.
Sign off: After you have collected each piece of evidence, hand them into your trainer / mentor who will then countersign them. Please note that they may ask for additional evidence or ask you follow up questions. Also ensure that:

You have date and signed your work

You only submit evidence for checking once it is completed (you may take individual evidence pieces out of your portfolio)

Ensure any countersigned checked work is returned back (in order) to your portfolio
Try not to hand in multiple pieces of evidence for marking at once - this will create too much work for your trainer / mentor, instead spread submissions out throughout your training.
Trainer notes: Ensure the candidates line manager completes the 'Line Manager's Comments' section as each module in the portfolio is completed - continuous assessment of the candidates work is required throughout the training period.
