Maintaining Standards of performance

Evidence 1: Complete the following table below explaining the 7 quality terms and their relevance to performance – it is recommended that you speak to your quality lead, training lead, mentor or line manager if you need any further explanation for any of the terms.

The following page form part I of the Certificate of Achievement may help for starters:

1. Quality terms and their meaning:

Maintaining Standards of performance, task 1

Evidence 2: Complete the table below by listing 6 quality documents and explaining the purpose of each in your own words.

Hint: Look under section 1 (Quality Management System) on the above link for examples (e.g. SOPs, policies etc.) of quality documents used in your scope of practice.

2. Quality documents used in your scope of practice:

Maintaining Standards of performance, task 2.

Evidence 3: Describe an audit you have undertaken that identified a finding or non-conformance (hint: use evidence piece 3 from the safety at work module).

3. Describe an audit you have undertaken:

  • Describe how you carried out this audit (list the steps taken).

  • List the findings or non-conformances raised from this audit.

  • Provide evidence that any one of the findings or non-conformances raised in the audit had been remedied / resolved.

Evidence 4: For this piece of evidence you are to raise a quality improvement for the department – this could be something raised at a departmental meeting, after discussion with your line manager or even as a result of an issue raised as part of an audit, CAPA or Datix.

Provide a piece of objective evidence of this improvement being discussed or implemented and place it within your portfolio (i.e. Quality Improvement Note, copy of meeting minutes, manager file note, updated SOP, improved training document/ photographic evidence etc.) Write on your evidence piece what the benefits are for your quality improvement.

Speak to your line manager to ask permission before taking any photographs in the laboratory.

Sign off: After you have collected each piece of evidence, hand them into your trainer / mentor who will then countersign them. Please note that they may ask for additional evidence or ask you follow up questions. Also ensure that:

  1. You have date and signed your work

  2. You only submit evidence for checking once it is completed (you may take individual evidence pieces out of your portfolio)

  3. Ensure any countersigned checked work is returned back (in order) to your portfolio

Try not to hand in multiple pieces of evidence for marking at once - this will create too much work for your trainer / mentor, instead spread submissions out throughout your training.

Trainer notes: Ensure the candidates line manager completes the 'Line Manager's Comments' section as each module in the portfolio is completed - continuous assessment of the candidates work is required throughout the training period.
