Evidence 1: Complete the table below demonstrating different storage requirements for 6 various samples / types.

Evidence 2, part 1: List the stages necessary to maintain the correct storage conditions for specimens (before and after testing).

Examples listed below may be applicable to your department – can you think of any others?

  • Correct labelling / identification

  • Physical space available

  • Identifying sample locations for recall / add on / referral

  • Temperature control and monitoring

  • Length of time stored

  • Preservative / fixatives considerations (e.g. ventilation)

  • Volumes required (e.g. sample aliquot of larger 24hr urines)

  • Secondary containers / lids used (e.g. parafilm once lid has been removed)

  • Cleaning regimes

  • Protection from light

2. Stages necessary to maintain the correct storage conditions for specimens:

Evidence 2, part 2: Situation: You need to defrost a freezer in your department that contains stored samples still requiring testing. Describe what considerations you would take into account before and after defrosting has taken place.

2. Defrosting sample storage freezer at work:

Considerations: 

  • Factors to consider before defrosting commences (e.g. alternative storage location, length of time to defrost, staff notification etc.)

  • Factors to consider during defrosting (Notices, water leakage, electrical safety, disabling Contronics etc.)

  • Factors to consider post defrosting (cleaning opportunity, temperature verification checks etc.)

Evidence 3: Complete the simple audit template below on a piece of temperature controlled storage equipment (or room) used to store specimens / reagents- try and choose a time when the temperature was out of range (e.g. door left open, opened to process samples, cleaning) so that you have some actions to complete.

You may use Contronics to assist you with this audit.

3. Audit on a piece of temperature controlled storage equipment:

Sign off: After you have collected each piece of evidence, hand them into your trainer / mentor who will then countersign them. Please note that they may ask for additional evidence or ask you follow up questions. Also ensure that:

 

  1. You have date and signed your work

  2. You only submit evidence for checking once it is completed (you may take individual evidence pieces out of your portfolio)

  3. Ensure any countersigned checked work is returned back (in order) to your portfolio 

 

Try not to hand in multiple pieces of evidence for marking at once - this will create too much work for your trainer / mentor, instead spread submissions out throughout your training.

Trainer notes: Ensure the candidates line manager completes the 'Line Manager's Comments' section as each module in the portfolio is completed - continuous assessment of the candidates work is required throughout the training period.
